Job description	Attention to Farshad AlimohamadiLAXThis is a supplemental permit to Permit No. 24016-10000-13320. The original floor opening was reviewed and approved under Permit No. 24016-10007-13320. During construction, the contractor slightly overcut the south edge of the floor opening, resulting in one leg of (13) #5 shear ties in the girder being cut near the southeast corner. To restore the lost shear capacity, the damaged girder will be repaired with a 3-sided CFRP wrap. This package identifies the damaged girder location and provides repair details. The repair will utilize the LOCTITE Tyfo CFRP System, designed to provide shear capacity equivalent to one leg of a #5 Grade 60 steel stirrup spaced at 5 inches on center. The CFRP wrap also provides additional confinement of the existing concrete girder top longitudinal bar.
